BACKGROUND CHECK FORM

As a service to our churches, FRBA has partnered with LifeWay to provide professional, convenient, confidential, fast, and accurate background checks as needed for church volunteers and potential employees.

An authorized representative of the church may print as many forms as needed. By completing the form and sending it to the FRBA office with a check of $10 per form, a search will be performed on your church's behalf and a copy mailed directly to your church's authorized representative. Whereas a search can be done by the local Sheriff's office for $7 per person, their search is limited to the state of Georgia. US OneSearch does a check of 49 states, Washington, DC, Puerto Rico and Guam.
